This list does not guarantee that your insurance will pay for your treatment. We will call your insurance company to verifity benefits as a courtesy. We do not accept any responsibility for wrong benefits notitfication by your insurance company. We advise you to call your insurance to re-verify your own insurance benefits. Your insurance is just that, your insurance. It is the patients' responsibility to ensure prompt payment for services either from the insurance company or personally. Any outstanding balances not paid by your insurance company will be billed directly to the patient to whom the services were rendered.
